Mountain Home Health Care, is 501c(3) non-profit organization serving Taos County, Colfax County, and northern Rio Arriba County by providing home health care services to those in need. As an approved Medicaid and Medicare provider, we offer a coordinated team approach to therapeutic and rehabilitative services – including Personal Care Services, physician directed Home Health Care (Medicaid and Medicare) and Hospice.

New Mexico AIDS Services is a nonprofit organization that advocates for and enhances the lives of people affected by HIV and AIDS by providing direct services and prevention education.
NMHEP, an initiative at the Santa Fe Community Foundation, focuses on building an infrastructure of community-based leadership that has the capacity and knowledge to change systems and address issues that make it possible for every New Mexican to have an opportunity live a healthy life. New Mexico needs residents actively involved and invested in making a difference, and it needs a strong set of community-based organizations that are effective and are a galvanizing force that is able to inform decision makers. HEP is building the foundation to make this possible.
Noesis Integrative Health is a State Approved Behavioral Health Agency that is Certified to provide Comprehensive Community Support Services (CCSS), Intensive Outpatient Treatment (IOP) as well as Medication Assisted Treatment with therapeutic services. Noesis Community Support Workers connect clients with supportive services within the community and maintain engagement with clients through their long term recovery goals.

Rio Grande ATP (RGATP) provides adult substance abuse intensive outpatient treatment program. RGATP focus is on the delivery of high quality, affordable, and accessible treatment services to consumers and their families. Services include case management and a central intake hub helping individuals get treatment that includes coordination of care for detox, residential, and transitional living services out side of Taos County (Golden Willow Counseling is RGATP’s community partner).
SafeTeen New Mexico is a youth driven non-profit organization. We partner with both public and private entities to fund and create programs that educate teens, parents, educators and community leaders about critical issues facing youth. Our programs include both In-School Assemblies as well as broadcast based Social Marketing Campaigns. Programs have included: distracted driving, underage drinking, drug abuse, date violence, body image, addiction, suicide and many others.
Southwest CARE Center, a model of excellence in the treatment of HIV, provides comprehensive medical care, research, education, prevention and support services to people affected by HIV/AIDS and related conditions in New Mexico.
Taos Alive Coalition unites the community to plan and implement strategies that prevent and reduce substance use and its consequences among Taos County youth.
Taos Elders and Neighbors Together (TENT) is a nonprofit 502(c)(3) tax-exempt community of elders and neighbors supporting one another. It is a virtual village with a volunteer-based support network that connects members to various resources needed for them to live comfortable, dignified and vibrant lives in their own homes as they age. TENT provides a variety of specific services, activities, and programs that further this purpose, including social activities, transportation, minor home repairs, caregiver relief, and much more.
